Metatarsal & Bunion

Metatarsal & Bunion

Don’t ignore this – it’s a signal!

Pressure in the metatarsal area is rarely random. It is often the first sign that the foot is losing its natural structure and load distribution.

If not addressed early, these changes can progress into Hallux valgus (bunion) – a structural deformity that, once developed, is typically corrected with surgery.

Note: At Dr. Obooi, we aim to educate our clientele with clear, easy-to-understand explanations of their individual situations. We believe that understanding your own feet is key to maintaining long-term mobility and comfort. By recognizing early signs and changes, you can take proactive steps to manage symptoms, slow progression, and make informed decisions about your care and treatment options.

What is Hallux valgus?

Hallux valgus is a common condition in which the big toe gradually bends inward, creating a visible bump on its outer side. It develops slowly over time due to changes in the structure of the foot. While it can run in families, its progression can often be slowed or managed with early intervention.

It is important to recognize its development stages, as it is an irreversible process; once fully developed, surgical correction is usually required (individual outcomes may vary).

Once you understand what even a persistant “pressure” in your forefoot could mean, you understand where it can lead and ways to support your feet and posture in order to reduce risk of progression.

Causes

Hallux valgus usually develops in people with an inherited foot structure that makes them more susceptible.

Tight or narrow shoes do not directly cause Hallux valgus, but they can accelerate its progression and trigger symptoms earlier.

Early signs to watch for:

  • Phase 1: Collapsed transverse arch → reduced shock absorption, calluses may form
  • Phase 2: Widening of the forefoot → increased pressure and pain
  • Phase 3: Big toe drifting inward → visible bump forms

Early intervention is crucial to slow progression and prevent more severe deformities.

Symptoms

Common symptoms include:

  • Pressure or swelling in the big toe
  • Burning sensation
  • Redness or swelling on the side of the toe
  • Occasional numbness

Symptoms often worsen with tight shoes or prolonged standing and walking.

Diagnosis of similar origin

A collapsed transverse arch can influence the structural balance of your foot and overall biomechanics. In simple terms, it may slightly alter how your weight is distributed while walking, which can change your gait and place strain on muscles and ligaments, potentially contributing to specific foot conditions.

Here are a few common conditions that can be related to changes in the transverse arch or overall foot mechanics:

What is Morton’s Neuroma?
Morton’s Neuroma is a thickening of tissue around the nerves between the toes, often causing tingling or sharp sensations in the forefoot. It can be triggered by pressure from a collapsed transverse arch or tight footwear, pressuring on the nerve, which can put pressure the nerves.

What is Achilles Tendonitis?
Achilles Tendonitis is inflammation of the Achilles tendon at the back of the heel, often causing stiffness or soreness. It can result from overuse, tight calf muscles, sudden activity changes, or altered walking patterns caused by compensation for other foot issues, which changes weight distribution and can strain the Achilles tendon.

What is Plantar Fasciitis and Heel Spur?
Plantar fasciitis is inflammation of the tissue (plantar fascia) along the bottom of the foot, often causing sharp or aching pain, especially with the first steps in the morning. Chronic plantar fasciitis can lead to the formation of a heel spur, a bony growth at the heel (calcification), due to prolonged tension on the plantar fascia. Both conditions may also be influenced by changes in gait or weight distribution caused by other foot imbalances, such as a collapsed transverse arch or Hallux valgus, etc.

What is Pes Cavus (High Arch)?
Pes cavus is a foot type characterized by an high arch, which concentrates weight and pressure on the heel and ball of the foot. This altered load distribution can increase the risk of strain, muscle fatigue, and secondary issues such as plantar fasciitis, metatarsal pain, or other biomechanical imbalances, etc.

How to determine your Diagnosis?

An orthopedic specialist or a podiatrist will examine your foot and usually confirm the condition with an X-ray. This helps assess severity and guide treatment.

Non-surgical Treatment

While the bunion cannot be reversed without surgery, treatment can help relieve symptoms and may slow progression:

  • Custom orthotics – supports the arch and may help address underlying foot mechanics
  • Proper footwear – wide, non-pointed shoes with low heels (max. 4 cm) to reduce pressure on the forefoot
  • Activity adjustment – reduce or avoid activities that trigger pain
  • Medication – anti-inflammatory drugs as prescribed by a doctor
  • Ice therapy – may help reduce swelling
  • Padding or toe alignment aids – can relieve pressure and provide temporary support

Important: All non-surgical approaches should be discussed with a qualified healthcare professional before use. Your doctor or podiatrist can help determine which options are appropriate for your specific condition.

"The Dr.Obooi Moment"

Dr. Obooi sandals combine footwear with orthotic-inspired support in one product. They are designed to support the transverse arch and help improve foot alignment during movement.

Unlike mass-produced shoes, every pair is custom-made for each individual, designed according to your foot specifics, comfort preferences, and daily activity needs. Each pair is intended to provide consistent support and assist in maintaining proper foot mechanics. ✅#WalkFree ✅#LiveBold

By providing consistent support, this may help reduce strain on the forefoot and contribute to slowing the progression of conditions such as Hallux valgus.

To get the best effect, the sandals should be worn as much as possible during the day - ideally around 80% of the time you are moving - and used regularly.

The sandals work naturally with your feet and are generally well tolerated. At the beginning, it is normal to feel some discomfort or pressure in the first 10-14 days as your feet adjust. The sandals do NOT function as a soft cushion, but as a supportive footbed.

Remember – the sandals are gently supporting your arches and applying counter-pressure to the areas that need it most. Once your feet adapt, and your arches are properly supported without collapsing or putting strain on ligaments, muscles, and nerves, you are supporting the underlying factors that may contribute to foot discomfort.

If you do not notice improvement over time, it may indicate that the root of the problem lies elsewhere, and exploring further options with a qualified healthcare professional is recommended.

When is Surgery Needed?

Surgery is considered if non-surgical treatments do not relieve pain.
The procedure aims to realign the bones, remove the bump, and restore normal foot function. Recovery time depends on the procedure performed.

Does Surgery Work?

Surgery can successfully correct the deformity, but Hallux valgus can recur.

Why can it come back?

Surgery corrects the structure of the foot, but it does not always remove the underlying causes - such as weak support, poor mechanics, or long-term overload.

How to maintain results...

To keep the benefits of surgery long-term, it is important to:

  • Wear supportive footwear consistently, like Dr.Obooi
  • Use orthotics as recommended, like Dr.Obooi
  • Follow recovery and strengthening guidelines, as advised by your healthcare professional
