But even 5 mm (about 0.2 inches) of difference can shift the balance of your hips, affect your lower back, and subtly strain knees, ankles, and other joints. Over time, uneven load can contribute to joint wear, ischemia, and long-term discomfort.

One simple way to notice leg length difference: stand naturally and look in a mirror at your shoulders. Uneven shoulders often reflect pelvic tilt caused by a shorter limb. This, in turn, can influence posture, gait, and balance.


Why Even Small Differences Matter

A shorter limb forces your body to adapt:

  • Pelvis tilts slightly, affecting hip alignment
  • Spine may curve to compensate, increasing stress on the lower back
  • Knee and ankle joints adjust differently on each side, which may lead to pain or overuse injuries
  • Muscles on one side may become tighter or weaker over time
  • Even minor differences can alter weight distribution and joint loading, gradually “eating away” at hips, knees, or ankles

A healthcare professional (orthopedist or physiotherapist) can:

  • Measure true leg length and functional differences
  • Assess gait, posture, and joint stress
  • Recommend imaging if structural differences are suspected

Non-Surgical Management

Even small differences can be managed effectively:

  • Custom footwear or orthotics: Helps balance leg length, supports arches, and improves gait
  • Stretching and strengthening exercises: Focus on hips, pelvis, and lower back
  • Posture awareness: Adjust sitting and standing positions
  • Activity modification: Avoid repetitive stress on one side

Important adaptation notes:

  • When starting with a new height correction, initial discomfort is normal. Unlike other conditions, your body hasn’t adapted to this balanced state yet.
  • If the difference exceeds 15 mm (~0.6 inches) and you haven’t previously worn a correction, begin with 5-10 mm less than the full adjustment, then upgrade to full height in a subsequent pair. Jumping straight to full correction may feel unnatural - like wearing only one shoe.
  • Expect a 10–14 day adaptation period, similar to other orthopedic conditions.
